file upload option

I am trying to make a form but I have no "file upload" option??  Can anyone help as I really need this ability for my website warranty so people can fill out a warranty card and upload photos

@IVANCCSOLAR  , file upload is available for Office 365 business and edu users. It hasn't been enabled for personal Microsoft account yet. Wondering what's the account type you are using?
