Hi,
So, I am from an Internal Audit Department. We are preparing a survey through MS. Forms which would be sent to different departments of our company after the audit of each department.
1. When I send the form to each department, I want a pre-determined audit name to be set on each form when the respective department opens their survey form. On the response sheet, I want this audit name to be unique from each responder.
Could anyone help?

@ahmedusam you can't pre-populate the audit name or any other information in a Microsoft Forms form; you would need a different form for each department. If you wanted all the responses from all departments collated into a single list you would need to build a flow in Power Automate that saves each response into a SharePoint list with a column that shows which department submitted it.
