Creating a Form that sends completion notification and results to individuals named within the form.

Hello,

I want to create a survey using Microsoft Forms and have the notification and result of a completed form go to a specific individual named/selected from a field within the form itself. 

Example:  A particular field on the form would ask the submitter to name or select their manager.  At the completion of the survey and once the form is completed, the completion notification and result of the survey would be emailed to the individual named/selected as the person’s manager.

Is this possible?

@OmarAngulo it's not possible using Forms on its own, you would need a flow in Power Automate. A simple example is shown below which sends an email to the manager and copies it to the person who submitted the form:
