I would like to convert my current excel form to Microsoft Form, can some one suggest me how do it since I am new to Forms.

@sunilkumar2245 you would have to that manually in Forms, there is no way to import that excel sheet into Forms. Alternativelya better way might be to look at a PowerApps form linked to a SharePoint list as the data source/repository. That way you  can have checkboxes and the form can replicate your current one.
