Blank rows in forms' answer record

Hi, everyone!

I have received some blank answers to my form, I can see who completed it and when he did it, however the rest of the fields are empty. This is strange because many of the fields are obligatory. 

Moreover, some minutes later, I received answers from the same people, now complete.

I would like to know why is it that forms submits empty answers since this questionary interacts with an approval flow.

I am also experiencing this issue. We shared a "duplicate" version of a form as a template, and all of the resulting Excel documents have the issue. Anyone have ideas on how to resolve this? Since the owners are all separate, I don't have the ability to sync their books, so I would like to resolve the issue with the template itself if possible.
