Add N/A option to Rating Questions

I would suggest to have N/A option in the rating questions and let designer to customize the label:

This is a needed functionality; we are saying Rate this if it applies; the only way I can see to do that now is allow them to skip the question. I would prefer to require the question and give them an opt-out "NA" option. Please
