Alerts with IP Address - N/A

We have High Priority alerts coming into CASB with an IP Address of "N/A".  What would cause this type of behavior?  Most of the access is coming in from the same VPN or network connections and and IP is registered for some activities but for some we get "N/A".  

Any information on this, please let me know.
