Covid-19 Health Declaration as part of the booking process

Hi,

I'm looking for a way to get people to fill out a Covid health questionnaire as they make a booking.  I've tried using Custom fields but I can't export that data to a spreadsheet so every calendar item needs to be checked individually.  Has anyone found a better way to do this?

Have you tried creating a "survey" that gets sent prior to the appointment? It looks like you can send one after an appointment so I don't see why you couldn't send one before.

Full disclosure, I'm new to Bookings so I don't know if this works. Once my access is granted I'll be trying this myself as we have a similar requirement.
