Is it possible to use survey response (Forms) to populate fields in a form?

I have a form that is used a lot, but there are lots of issues getting people to fill it out correctly. I want to create a survey in Forms and use the responses to autofill the form so it can be printed and signed. Is this possible in MS Forms?
