Recipient Specific Email Form

I am looking for a way to create an email form that generates for a specific recipient for any user in the organization. So, for example, if any user enters admin@ourcompany.com in the To: or CC: field the body automatically populates a form with drop-down menus and text fields.

You have to program a bit to do what you want. Create a macro/add-n yourself or hire someone.
