Taking information from a PDF and creating a spreadsheet

Is there a way to take information from a uniformly formatted pdf document (https://hnldoc.ehawaii.gov/hnldoc/document-download?id=11478) and convert it to a spreadsheet. Ideally, I need fields for Name, Phone, Email, Meeting Date, Position on the Matter, and the Written Testimony. The pdf is 300+ pages and it would be nice to standardize the data entry if it is possible on excel.
