Question related to return duplicate value

Hello Everyone, 

Tried the formula and it works for FIRST NAME

But when i have done autofill, i got same value

like - 

So what should i write formula to return all values and it should be dynamic?

Please help..???

@Excel You should index the entire Data range and add a MATCH to find the correct column number. See attached.
