Pivot Table Assistance

Hello there Experts!

I have a *I believe* a pivot table question. I'm looking to create a table that generates a cost when answering five questions. Each question would have to be linked to a data sheet that has the data to pull from. My question is: When generating this program in excel, which formulas would I have to use to do that, and how would I use them? Please refer to the options below. I'd like it to be a table that I can select options from or type in a response and it associates the response to the data on the additional data sheets.
