Need Help with Formula complex

See attached picture.  

I need column G 'Referral Date' text to turn Bold when column E does not have a 'Yes' documented in 3 days and red after 5 days. 

But also need...

the background of cell in column G 'referral date' to turn color A if column J is empty after 3 and color B if blank after 5 days. 

Huge help. I hardly knew where to start here.

you need to setup a set of rules for conditional formatting:

In the attached Excel file you will find my example. I'm not sure if it meets all requirments but it should give you at least a good start.
