So this is an excel forum where we can help you with issues you are having with excel. If you can't find a template in Excel Open dialogue or the download libraries, I suggest you try some google searching.
That said, there are many many FMEA templates and don't necessarily use excel. At some of my companies we use them in excel and others do not. It all depends on what are your policy requirements (and/or regulatory requirements) . I deal with ISO13485 (which is medical analogy to the ISO 9001) and ISO 14971 (Risk Assessment). based on the different product risk levels at the different companies require different approaches. I just hope you will find a process that meets your needs and not just implement a process that may be overly burdensome for what you need.
