Move or Copy worksheet containing Form control

Odd issue I'm having, I have a template worksheet for data capture, every week the "blank" template is copied using the Move or Copy function. I have added to the template a form control button (with macro) which will pull data entered into a weeks worksheet and store in a historical data sheet. 

When I copy the "blank", the form control button doesn't copy with. This is working on other similar files and I can not figure out why.

With your permission, add a file (with no sensitive data) from which you can accurately describe the problem. That way you can be helped faster.

At the same time, the helper can find the sources of error better and faster in the file and offer you the best possible solution.
