MIP File will not pull info from a 2nd tab into the summery sheet

I have a Data Base system that that uses MIP technology to create a CSV file.  In the template MIP file I have built in Cell references across 2 tabs.  When the MIP file is created the data is showing on the second tab (sheet 2) but does not pull in to the summary sheet (Sheet 1)even those the cell reference is correct and is showing on the 2nd tab.  Can any one help.  I have uploaded the template MIP file (RFI Log) and the out put CSV File (MIP Output).  The cell reference in question is Sheet 1!B% and Sheet1!C5.

If you import csv file it'll be no any references within it, as well as any other formulas. That is just text file.
