Index Match and tables issue

Not sure what is wrong here. I have an index match command that works fine. My tables are set up correctly. When I add a record to the table, all commands and formua are appended EXCEPT my index match commands. What am I doing wrong?

Do you use structured references? Perhaps you may illustrate with sample file or at least with screenshot.

This formula appends when I add a record.

=IF(B217="","",[@[Date completed]]-[@[Date received]])

This one doesn't:

=INDEX(Table12[#All],MATCH([@Description],Table12[[#All],[Description]],"0"),MATCH(Workorders[[#Headers],[Value/piece($)]],Table12[#Headers],"0"))
Table12 is on a different worksheet.

Here is a portion of the table that I need to append as new orders come in. I use the excel form since I haven't set up my own yet.
For some reason I can't add a screenshot here. Let me work on that.
