I Need help creating a data page that will let me select a group of serial numbers that will print on a monthly log.

For Example I have 20 pieces of equipment That need monthly logs for different Programs.

I want to open the Excel Document, enter program name and new month and print each log with the individual serial number and different program areas. The Log has already been made, I just need to know how to get the data to insert into the document and print each page with each serial number. Like printing a set of logs.

besides showing the form where the data need to be inserted, you also need to show the dataset or a sample of it in order to formulate a solution for example:
1+a
you showed one value but we don't know the value of "a"  (which is your dataset)
