Formula dependent invoicing system

I'm trying to come up with a better invoicing system for our organization's dues collections. We have a dues calculation that is based on our members revenue amount. Right now we send them a paper invoice, they enter their revenue amount, and then look at the schedule to see what their amount due is and mails us the statement and a check.

What I want to do is offer them a link to a web-based spreadsheet. They enter their revenue amount, the dues are calculated for them and then links to our online payment portal. I need to be able to access that revenue amount and dues amount after the fact though. Any ideas on a good way to do this??

I also want to have a check box if they pay by credit card that increases the dues by a convenience fee.
