I think there may be some confusion as to what's expected for the desired results.  The codes are unique but a given code is shown 3x in the desired results:

Shall all codes be shown 3x each in the desired results? Additionally, it appears the 3rd stack in blue shows multiple codes whereas the first two stacks repeated a given code 3x.

Sorry for the confusion. i think the below table explain more (am just to trying to seperate the codes to be parents and sub through levels according to the combination code)
