We have been working on a document for several hours, had autosave turned on, and were also manually saving as we went. I closed the document for a bit, and when I reopened it, it was not the latest version, but instead a version from 6 hours ago. This is the second time this has happened, now.

A guess: you may be inadvertently saving it to a different folder or directory when you do it manually.

I've done that, accidentally, more than once. If you ask Excel to "Open Recent" it may come up with two or more files with the same name but different locations.
