Every time I try to use the INDEX and MATCH feature I get a #REF error.  I am working on a very big document.  Even if I create a new table, from scratch and try to Index/Match something I get that error.

In general such formula works

Perhaps you have conflict with some other names in your workbook. It's suspicious that "Index" is in low case in formula bar. At least you have sheet Index, but that shall not affect. Maybe something else.
