conditional formatting

I have two worksheets within the same workbook - that I have applied conditional formatting to.  However, when I save the work book - only one of the worksheets saves the parameters I've set up - the other loses the formatting.  Is there something I need to do - to have both sheets save the information.

The conditional formatting applied to the two sheets should obey..

However,, if you're still having the problem, you can upload a sample file for contributors here to takes look at it

it works if I change the type of conditional format, one page has icons and the other colors, but using icons on both doesn't work!  very weird.

where would I find a sample....i'm new to this site.
