If I used service default MFA and enforce MFA with Microsoft auth app would it only apply to Azure portal based access or it could be applied to on prem app using azure app for authentication or AD?

Where did you enable MFA? In the Conditional Access Policies?
How did you configure it?
When it is a Conditional Access Policy, it will apply for various Stuff tied to the AAD.
