'open file' page not showing second last file

A recent scenario keeps surfacing;  When opening Open file page, the list is not displaying all of the saved files. 

When I save a file, I save a duplicate in second  location, at the same time. The backed up file displays but not always the saved file in its original directory. 

Am I doing something wrong?

If you're opening a file, it will appear in Recent list. And if you Save a copy it will also show in the list. And because you see it intermittently then it's certainly unusual.

Out of curiosity, are you using SharePoint or OneDrive to store your files? Version history will retain your files so you needn't save a separate copy as you can always view/restore a previous copy if you need content.
