Cannot access responses

I have been using Microsoft Forms for many months. Today, suddenly, I cannot access the responses. A pink box at the top of the screen says: "This form can't be distributed as it is asking for personal or sensitive information." I need to access these responses, how can this get unlocked?

@jimp50I recently got the same issue with my form.

It was working for several weeks and now it's blocked.

What's funny that I created my survey based on one of shared templates.
