Formatting a drop down list

Good Evening,

I am in desperate need of some help to complete a form for the charity pre-school where i work.

 I have attached the sheet in the hope that someone could help. I am stuck on ow to make the drop down list change colour and show in the corresponding column at the bottom and to the right. 

I hope someone can help :)

You should help first on understanding question.

Make clear "What you want? & Where you want the result?" . In excel term, explain it.

Specify row / column names.
