Excel data format for Power BI

Greetings All,

 

See attachment for data. I am looking to organize the data to fit into Power BI (my first project). The data comes from a paper survey that is being manually entered into Excel. The auditor has to survey food delivery bins to ensure that they comply with the 5 questions being asked. (Y) for yes they meet, (N) for no they do not meet. How can I calculate the Ys and Ns to show a graphical presentation of the data? Do I need to reformat the data? Any help would be appreciated and thanks in advance.
