Lost Audio output after Windows update

I lost sound on my ThinkPad T490 after the last update over the weekend. I resintalled the update two times. Ran troubleshoot a few times with no luck. I uninstalled the Realtek driver and downloaded a newer version, it did not work. I uninstalled it and installed an old version and still no working. I keep getting the error "Apparently, your device audio services are not responding."

I am running Windows 11, This is the latest update Windows 11 Insider Preview 27766.1000 (rs_prerelease)

Processor Intel(R) Core(TM) i5-8265U CPU @ 1.60GHz 1.80 GHz
Installed RAM 8.00 GB (7.75 GB usable)

the device manager is fine. The Audio items are displayed properly and the Sound items are all fine

 All the Windows Audio services are running, same as the realtek service

    Sometimes Windows likes to mess with your settings. Double-check that the correct audio output device is selected. Right-click on the sound icon in the taskbar, click on "Open Sound settings," and ensure your output device is set to the correct option.

    I am facing the exact same problem since this afternoon. This morning my audio was working fine, I inserted headphones and the Realtek audio stopped working. Removed and reinstalled all drivers to no avail. I restored my ThinkPad from an image taken this morning and the internal audio worked again but the headphone output didn't. Now neither work again. Troubleshooter says "Apparently, your device audio services are not responding." and the test sound fails to play through either Realtek outputs.

    Edit: I restored an image of my laptop from end of November (before the Windows 24H2 update), installed all latest updates and drivers (including 24H2) from Windows Update, installed latest Graphics and Realtek audio drivers from Lenovo and everything is working fine. Sound plays either from the internal speakers or headphones if I plug those in. Before, I had to manually choose the second audio device to get the sound to play from headphones. I cannot figure out what the issue was, since to me it looks like I am back at the same state I was in when the problem happened!
