Duplicate Identical Appointments

We have a teacher who has setup a Bookings page.  Students book times with her for phone calls or visits.  One student has booked an appointment on two different occasions and the appointment shows up twice in her notifications and shows twice on her calendar.  They are always for the exact same time and date.  I am enclosing screenshots.

What could be causing this?
