Customer name disappears in the calendar view

When I create an appointment, I add the customer name, email and phone number. However, after the appointment is created, when I look in the calendar, the customer name has disappeared. 

I have verified that the name exists correctly in the Customer list.

Any idea what I am doing wrong?
